This movie illustrates a duplex ultrasonographic examination of a native arteriovenous fistula (anastomosis in the elbow between the brachial artery and the cephalic vein).
A stenosis on the venous run-off (in the perianastomotic area) is depicted.
The flow in the brachial artery proximal to the anastomosis is laminar (Colour Flow mode)
Examination with spectral Doppler analysis reveals a low resistance flow pattern.
This is a normal finding in the feeding artery proximal to the anastomosis and exludes fistula thrombosis. A decrease in the diastolic flow component, is a sign of (impending) thrombosis.
- Just after the anastomosis, the cephalic vein has a narrowed and irregular aspect in B mode imaging.
- In Colour Flow Mode, aliasing (the confetti phenomenon) is seen inside the blood vessel and outside aswell (due to tissue vibration). Those are signs of high and turbulent blood flows and indicative of stenosis.
- With spectral Doppler, high flow velocities are recorded (upto 250 cm/sec). When the blood flow velocities in a suspected area are 2-3 times higher, compared to a non stenosed vascular segment proximal or distal to it, a stenosis is suspected.
Based on 1,2 and 3, this patient has a venous stenosis in the perianastomotic region of his fistula. Whether this stenosis is significant (with danger of losing the fistula) and needs further workup and treatment, can be a matter of debate.
In this case a “watchful waiting” approach seems most appropriate.
Some parameters need to be followed closely in time (pressure at dialysis needles, kt/V, ease of fistula cannulation, volume flow through fistula,…)
When a negative evolution is observed in one of those parameters, further investigation with angiography is necessary to confirm the diagnosis and exclude another problem (fex central stenosis). When the decision has been made to treat , then a surgical treatment is first choice (excision of stenosed segment and proximalisation of the anastomosis) rather then PTA (interventional radiology).
